Experimental treatment- 100% actual charges up to 4k per calender year, stem cell or bone marrow transplant 100% of actual charges to 10k/ life. Surgery- 100-$5k per procedure. Hospital confinement of $350 day for up to 75 days. Private duty nursing of actual charges up to $100 day for up to 60 days. Home healthcare- actual charges up to $100 day for up to 60 days. Hospice care- actual charges up to $100 day for up to 120 days per lifetime.
